Free application for birth certificate

This program is for persons born in Oregon. Check with the state where you were born to see if they offer free birth certificates to persons who are homeless. Please contact a participating homeless service provider or organization to get started. A list of acceptable proofs of identity is on the Oregon Vital Records website. Participating homeless service providers and organizations can help you get your acceptable proofs of identity.

The grant program was established for individuals who are homeless to get their own birth certificate free of charge.

Your birth certificate will be mailed to the address on your order form. The envelope will be addressed to you since it is your birth certificate. A birth certificate is a legal document used to establish identity. It shows who you are, and when and where you were born. Your birth certificate is a legal document and is confidential.

How do I get my certificate?

Please see the Vital Records Reference by State to find out where to obtain birth certificates for other states. To request a New Mexico birth certificate, please continue reading below. We do not accept credit cards or online orders; however, for your convenience, you may apply online through VitalChek, an independent company that we have partnered with to provide you this service. We are now offering two options for ordering certificates through VitalChek.

Additional service and shipping fees apply. When requesting a birth certificate via the mail, you can either fill out the form and mail it in, or send a letter with specific information. Print out the Birth Record Search Application form and fill out all required fields. We also have a Spanish Version of the form. Instead of filling out the form, you can alternately write a letter with the following information.

If you receive your mail at a post office box, please include a physical street address for our internal files. Please make your certified check or money order payable to "New Mexico Vital Records" in the exact amount. Do not send cash. Same day service is not guaranteed for requests such as paternity affidavits, adoption processing, corrections and amendments. An original, certified copy no photocopies of the registrant's death record must be submitted with the request.

There are no exceptions. The procedures and requirements depend upon the specific information to be changed and whether any previous changes have been made. Again, you should indicate the correction you wish to make.

No Fee Birth Certificate for Homeless Persons

How to Get a Copy of a Birth Certificate

A delayed birth record can be established if 3 different documents verifying the birth facts claimed are submitted. The documents required are based on the age of the person whose birth certificate is being requested at the time of the application.

For a person more than ten years of age, the documents must have been established at least ten years prior to the date of application. For a person ten years or younger, the documents must be dated at least one year prior to the date of the application or within the first year of life.
